I think the hospital job was available earlier but I only just discovered it and as a gay man I have no interest in the women at all.

Secondly, it's not just a sissy game, there is a cis-het path where you stay masculine and do masculine / dominant things to cis women, and there's plenty of content, test all 3 options when the game tells you to save. The only problem is I made 5 save files and now they are lost after logging in the next day. That is incredibly frustrating, because that was probably about 3 hours of progress to get to that point.
